How to complete the Online Feedback for LMSC 2010?

Please follow the 6 steps mentioned below to complete the online feedback for the LMSC 2010.

STEP 1:

Click on the link: http://spreadsheets.google.com/viewform?formkey=dHptZ0h5RklLRkduakdib3hPTjBhY0E6MQ

STEP 2:

Proceed to Question 1, where you will be required to enter your name. Please be reminded to provide your FULL NAME (e.g. John Tan Ah Kow instead of John).

STEP 3:

Proceed to Question 2, where you will choose your school from the drop down menu. Choose carefully so that your entry will be properly recorded.

STEP 4:

Proceed to Questions 3 and 4. Please answer either YES or NO for both questions. Questions 3a and 4a are provided for additional comments on Questions 3 and 4 respectively.

STEP 5:

Questions 5, 6 and 7 are for the Effective Communication and MOI module. Please answer them based on the lecture contents and activities. The questions are based on a scale of 1 to 6, with 1 representing Strongly Disagree and 6 representing Strongly Agree.

STEP 6:

Questions 8, 9 and 10 are for the Leadership module. Please answer them based on the lecture contents and activities. The questions are based on a scale of 1 to 6, with 1 representing Strongly Disagree and 6 representing Strongly Agree.

STEP 7:

Questions 11, 12 and 13 are for the Team Building module. Please answer them based on the lecture contents and activities. The questions are based on a scale of 1 to 6, with 1 representing Strongly Disagree and 6 representing Strongly Agree.

STEP 8:

Questions 14, 15 and 16 are for the Goal Setting module. Please answer them based on the lecture contents and activities. The questions are based on a scale of 1 to 6, with 1 representing Strongly Disagree and 6 representing Strongly Agree.

STEP 9:

Question 17 is also based on a scale of 1 to 6. If your rating is below 3, please provide areas of improvement in the box provided below.

STEP 10:

Once you have completed the form, submit the form by clicking on the "Submit" button at the bottom left hand corner of the page.

If you have any problems completing the Online Feedback, please leave a comment at the post with your Name and School, and we will get back to you as soon as we can.

Note that completing the online feedback will help the instructors improve on the course for the future batches of participants. Thus, your honest feedback is greatly appreciated.

Also note that it is COMPULSORY for everyone to complete the online feedback. We will collate the names of participants who do not complete the feedback form and reflect this to their Teacher Officers.
